The Ultimate Silhouettplasty

Motherhood, for most women, is a very joyous process. The toll pregnancy takes on a mother’s body is the cost of a growing family – and well worth it! But when a woman is done having children, her body often has a new shape, and she can experience medical conditions that interfere with daily life.

It is becoming increasingly common for women today to be aware of the potential for restoring their pre-pregnancy body through aesthetic plastic surgery. Thanks to the innovative plastic surgery industry, what was once deemed impossible can now be achieved. And now, we’ve reached an even more amazing milestone with Dr. Larry Schlesinger’s Ultimate Silhouettplasty.

This state-of-the-art procedure is pioneered by the Breast Implant Center of Hawaii, and is changing lives for those who look to enhance their body to a level they may have never even imagined was possible.

Those interested in such procedures can readily educate themselves on the available options, optimal techniques, and remarkable results with the information below. Dr. Schlesinger also recommends sites like RealSelf.com if you’re looking for in-depth details and more research.

What is The Ultimate Silhouettplasty?

The Ultimate Silhouettplasty in Hawaii is a new Tummy Tuck procedure developed by Dr. Larry Schlesinger and delivers the ultimate results. This new approach combines Abdominoplasty with Liposuction and other complementary procedures, customized to each patient’s unique needs and desires.

Abdominoplasties, commonly referred to as Tummy Tucks were originally developed as a reconstructive surgical procedure to address some of the most undesirable physical effects of pregnancy. During pregnancy, the growing baby puts pressure on the abdominal skin, causing it to stretch. Additionally, eating for two often leads to the accumulation of fatty deposits in areas where they did not previously exist. These factors, combined with pregnancy weight gain, can result in a range of aesthetic and functional issues.

The midline abdominal muscles, known as the rectus abdominus, separate to make room for the uterus as it grows. Failure of this separation could cause the uterus to move upward, potentially leading to significant health problems for the mother. Unfortunately, once the rectus abdominus muscles separate, they rarely return to their original position. As a result, abdominal organs can push forward, causing lower and sometimes upper abdominal protrusion. Furthermore, these separated muscles are unable to counterbalance the back muscles, often leading to chronic low back pain in women who have had one or more pregnancies.

Post-pregnancy, the stretched-out abdominal skin often attempts to contract back to its pre-pregnancy state but the elastin fibers in the skin are frequently stretched beyond their elastic potential. As a result, loose skin hangs downward, causing the vulva containing the external female organs to sink out of its natural position towards the inner thighs. This can lead to stress incontinence and a less favorable position of clitoral friction during intercourse.

Fortunately, Tummy Tucks combined with Liposuction have been shown to correct many of these post-pregnancy conditions. Early-generation tummy tucks tightened the skin, repaired separated musculature, and removed some of the newly deposited fat in the love handle or muffin top area. However, such Liposuction must be conservative to avoid compromising the blood supply to the entire abdomen.

The abdominal skin receives its blood supply from the muscles below, in a vertical fashion, through muscular cutaneous perforators. After an Abdominoplasty, most of these vessels are cut, and the new blood supply to the abdominal skin comes from the periphery in a horizontal fashion throughout the entire abdominal region. This new horizontal abdominal blood supply appears to be intact and functional within the first 90 days after the Abdominoplasty. Below begins to lay out what then began our road toward the conception of The Ultimate Silhouettplasty.

What Led Us to The Ultimate Silhouettplasty?

In recent years, Abdominoplasties have gained popularity as an aesthetic surgery, and not just as a reconstructive procedure. This trend has been fueled in part by websites such as RealSelf, where prospective patients can explore the possibilities for stunning body transformations and discuss procedures with both surgeons and actual patients. A successful aesthetic Tummy Tuck involves the tight repair of abdominal muscles both above and below the belly button, resulting in a flat and toned abdomen. Skilled surgeons take great care in creating aesthetically pleasing belly buttons on these newly flattened surfaces. In the best-case scenario, the final Abdominoplasty scar is fine, thin, and concealed beneath most bikini bottoms. Most other plastic surgeons will also include Liposuction of the lumbar roll or love handles in their abdominoplasty procedures. However, sometimes small out pouches of skin, known as dog ears, can occur at either end of the incision. This outcome often correlates with the amount of residual subcutaneous fat remaining after surgery, as well as any scarring from the Liposuctioning of this area. Despite wearing a girdle or undergoing post-operative massages or ultrasounds, some residual scar tissue may still remain, leading to a thicker waistline than may be ultimately achievable.

In a perfect world, a slender patient and a highly talented and motivated Board Certified plastic surgeon can achieve spectacular results after one procedure. But what about everyone else? With The Ultimate Silhouettplasty, a second procedure is added at 90 days. This is aggressive Liposuction of the remaining abdominal fat which includes fat in the tissue previously above the belly button and also residual fat and scar tissue at the waist. The Ultimate Silhouettplasty leads to a thinner and more delicate draping of the abdominal skin over a woman’s torso. This second stage of Liposuction surgery of the abdomen could not be done safely with the original procedure because the new horizontal blood supply to the skin did not exist.

The improvement of the blood supply to the abdominal skin is known as a “delay phenomenon” in plastic surgical terms. The original abdominoplasty withdrew much of the abdominal skin vasculature, and in so doing taught the skin and subcutaneous tissue to live on a new blood supply. As the new blood supply is well established at 90 days, it would not be compromised by this second Liposuction.

Why do a second stage to a successful abdominoplasty procedure?

For many women, although the results from a primary abdominoplasty are good, they had hoped for more. The Ultimate Silhouettplasty allows the average woman with excess fat deposits and an average waistline to achieve incredible results equal to or better than the most beautiful primary abdominoplasty outcomes. This second procedure also allows Dr. Schlesinger to eliminate minor dog ears that may have occurred and turn an acceptable abdominoplasty into a significantly better silhouette.

For patients with a BMI (body mass index) of 30 or higher, Dr. Schlesinger can reverse this process and perform Liposuction first to help “jumpstart” natural weight loss, helping heavier patients more easily reach their healthy goal weight and qualify for a safe Tummy Tuck procedure.

A New Approach to Tummy Tucks and Mommy Makeovers

Aesthetic surgery is a process, not a one-time event. Plastic surgeons and patients alike would prefer a single procedure, but it is often not possible. After all, it took nine months to create a post-pregnancy body, and it is unrealistic to expect a full repair in just one four-hour surgery. However, with proper planning and timing, good plastic surgeons can safely achieve the desired outcome.

What are the Benefits of The Ultimate Silhouettplasty?

Our patients in Hawaii who undergo a Tummy Tuck have experienced the following advantages of the procedure:

  • Enhanced physical appearance and increased self-confidence
  • Improved control over body movement and mobility
  • Increased capacity for more intense physical activity due to the elimination of excess skin
  • Reduced risk of certain medical conditions and prevention of painful hernias
  • Greater motivation to achieve better health and weight loss goals
  • Relief from stress urinary incontinence (which some women may experience after pregnancy)
  • Strengthening of the abdominal muscles and restoration of a stronger core
  • Enhanced flexibility and overall physical fitness

Who are the Best Candidates for The Ultimate Silhouettplasty?

The ideal candidates for The Ultimate Silhouettplasty are those who have weakened  abdominal muscles due to significant weight loss, pregnancy, or aging. This procedure is especially beneficial for individuals with excess skin and sagging folds in the abdominal area. Patients with less fat accumulation and more skin elasticity may be better suited for a mini-tummy tuck or a less-invasive Liposuction treatment instead of a full Ultimate Silhouettplasty.

What to Expect on the Day of The Ultimate Silhouettplasty

The Ultimate Silhouettplasty is very unique to each patient, and the procedure typically takes around 2 to 5 hours and is performed under general anesthesia. Dr. Schlesinger will typically make an incision from hipbone to hipbone, just above the pubic area, as well as a smaller incision to free the belly button from the surrounding tissue. Next, he will separate the skin from the abdominal wall and lift a large skin flap to tighten the vertical abdominal muscles, resulting in a firmer abdominal wall and a narrower waistline. The excess skin is then removed, and a new hole is created for the navel, which is then sutured in place.

The Ultimate Silhouettplasty includes an additional procedure at 90 days which involves aggressive Liposuction of the remaining abdominal fat, including the fat in the tissue above the belly button and residual fat and scar tissue at the waist. This second procedure results in a thinner and more delicate draping of the abdominal skin of the torso. The reason this second-stage Liposuction surgery could not be performed safely during the initial procedure is that the new horizontal blood supply to the skin did not exist. Each experience of Ultimate Silhouettplasty can vary with the patient’s needs, to get a better understanding of what your day will look like for your Ultimate Silhouettplasty, please call for a consultation with our team.

The Ultimate Silhouettplasty Recovery

The recovery period for the Ultimate Silhouettplasty Tummy Tuck is relatively long, but a second procedure, such as abdominal Liposuction, can have the patient back in the gym within six days and back to work within three to four days for many patients. The best plastic surgeons understand how the body heals and how to achieve optimal results safely and beautifully with appropriate time gaps between procedures. While some women may be satisfied with a standard 21st century Tummy Tuck, more and more are seeking the ultimate result: The Ultimate Silhouettplasty. This new approach combines abdominoplasty with Liposuction and other complementary procedures, customized to each patient’s unique needs and desires.
