Does Male Breast Reduction Surgery Shrink the Nipple Areola Complex?

Larry Schlesinger, MD, FACS explains why gynecomastia surgery will reduce the size of the nipple as well as what to do if a man wants further reduction.

Hi, I’m Larry Schlesinger, and I’m a board certified plastic surgeon from Honolulu, Hawaii. The question today is “Will gynecomastia surgery reduce the size of the nipple?”

I’m assuming the question really means the nipple areolar complex which is both the nipple and the areola. Normally, the male breast tissue stretches out (and fat of course), the nipple and so when you reduce the gland and the amount of fat present, you also reduce the nipple. I have seen this happen over and over again. When the gland is gone the nipple areolar complex becomes smaller. However, I am certain there are cases out there where it doesn’t. My experience is that this is a good use for the Benelli mastopexy. You decrease the size of the areola. You take an outer circle and an inner circle, you remove the skin, and you sew it up.

My personal opinion is that you should not try a Benelli mastopexy at the same time as a gynecomastia surgery because when you do the gynecomastia surgery, you’re going to aggressively liposuction, you’re going to cut gland out from below the nipple, and when you do that, you definitely do not want to then, start de-epithelializing the areola because the problem becomes – where is the blood supply?

At the Breast Implant Center of Hawaii, we break this into two separate surgeries. One, get rid of the gynecomastia, get rid of the gland and the fat. And two, do the nipple areola reduction.
