OK, so the first thing we’re going to do today is we’re going to do some tumescent liposuction. This is a cannula that allows water to go in, we’re putting salt water in, it’s salt water, sodium chloride with one thousand epinephrin per liter of saline. Which gives us 1:1,000,000 concentration.
Tumescent means to make rigid with fluid and this was suggested by Dr. Klein of Californina. And this has turned out to be a very useful adjunct to liposuction. One of the things you can do here is to kind of encourage the fluid to go through the different layers. It’s a form of massage in Hawaii, it’s lomilomi. So we’re lomilomi her hips. But it’s working already you see, this is turning whiter. And so now I’m going to liposuction this and with a little luck we’ll get mostly fat and very little blood. At least that’s the object of the exercise. This is a very small hole which we call a puka in Hawaiian and we’re going to sew this up, under the skin so it’s called subcuticular. And this is called pre-tunneling. We determine where we’re going to go and we kind of undermine it a little bit so that we can get there easily. Pre-tunneling.
And the cannula is hooked to tubing that is hooked to a liposuction machine. This one was made by Gunter-Graham. And here we go, lipo.See here it is. Very little blood, mostly fat. You’re whittling down the fat, which will make this young lady very happy.
Now we’ve done a nice job of taking down the elevation created by the fat. We don’t want to make a dent so we’re going to take this, we’re going to tighten this and that will make a big deal of difference. And then ultimately we come back at three months, this being the backside of a lower body lift, this being liposuctioned. We do the tummy tuck when we come back at three months and liposuction again everything, that is ultimate silouetteoplasty.
